Recognition Forms:

These new forms have been redesigned to make them easier to fill in and are able to be saved using Adobe Reader. Handwritten forms are not acceptable due to the loss of clarity when copied. Drop down boxes have been set for Councils to insert their Council name and the check boxes for the various awards are easily checked off by clicking on the applicable box. Council signatories are no longer required; instead we ask that the designated individual inserts their name which is their consent that this form is approved by them.

Please consult the Outstanding Service Awards process to "Thank" a volunteer and nominate them for an Outstanding Service Award, after the 2012-2013 Scouting year the Outstanding Service Award forms will not be used. 

The Meritorious Conduct and Gallantry forms have been combined into one form requesting the applicable information. This ensures that the appropriate award will be assigned and forms won’t have to be redone if the wrong form is used. The same applies to the Award for Fortitude and the Jack Cornwell Decoration. Award for Outstanding Service to Scouting (Certificate for Commendation, Medal for Good Service, Medal/Bar to the Medal of Merit, Silver Acorn/Bar to the Silver Acorn, Silver Wolf and Silver Fox)
